Case Managers

The Trauma Burn Program has two case managers.

The Outpatient Case Manager is Trish Applegate. She is mainly responsible for the day-to-day function of the clinic and follow up on all patients that have been cared for by the Trauma Burn Service. These functions include providing telephone triage for patients, outside physicians, and facilities, providing patient care, and patient education. She is also responsible for orientation of residents and medical students to the Trauma Burn Clinic.

The Inpatient Case Manager is Britt Elder-Tennyson. She acts as a liaison between members of the multidisciplinary team, which includes but is not limited to physical therapy, occupational therapy, Physical Medicine and Rehabilitation, social work and practice management. She rounds with the physicians in the morning and gathers data/information regarding the patient status. This information is then shared with and reciprocated from the appropriate team member(s).Her responsibilities include data collection, protocol monitoring and compliance. The inpatient case manager is at the coordinating center of the multidisciplinary team process to assist and assure resource management and appropriate discharge plans and needs. She is also responsible for orientation of the residents and medical students to the Trauma Burn Service.
